Pediatric Globe Rupture

  • Etiology: Trauma to eye
  • Imaging CT:
    — Loss of normal spherical shape of eye
    — Make sure to evaluate the lens
    — CT less sensitive than clinical exam to detecting injury
  • DDX:

  • Clinical: Serious injury associated with high frequency of vision loss

Axial CT without contrast of the orbits shows loss of the normal spherical shape of the left globe with some fluid present anteromedial to it. The left lens is not visualized. In the operating room there was a defect in the anteromedial aspect of the left globe

Axial (above) and sagittal (below) CT with contrast of the orbits show a left open globe injury with hemorrhage in the posterior aspect of the left globe and left anterior chamber collapse with abnormal configuration of the the left lens. On opthalmologic exam there was also left anterior chamber injury and left vitreous hemorrhage.
