Pediatric Ovarian Granulosa Tumor

  • Etiology: Malignant sex cord-stromal tumor that is an estrogen secreting functional tumor
  • Imaging US:
    — Solid-cystic sponge-like ovarian mass – solid mass with variable cystic foci or predominantly cystic with small solid foci
    — Enlarged uterus with endometrial hyperplasia or polyps or carcinomas
  • DDX:
  • Complications:
  • Treatment: Surgical
  • Clinical:
    — Presentation is precocious puberty due to estradiol secretion in 82%, menorrhagia or amenorrhea, virilization in 6%
    — Most common hormonally active tumor
    — Seen in first two decades of life – 44% from 0-10 years and 34% from 11-20 years
    — 10% of all precocious puberty cases
    — Associated with Ollier Disease and Maffuci Syndrome
