Hashimoto Thyroiditis

  • Etiology: Autoimmune thyroiditis
  • Imaging US:
    — Initially thyroid gland diffusely enlarged with heterogenous echotexture
    — Hypoechoic micronodules with surrounding echogenic septations or hyperechoic micronodules with surrounding hypoechoic septations (pseudonodular pattern)
  • Imaging US Color Doppler: Usually normal or decreased flow on US Color Doppler but occasionally may be hypervascular
  • DDX:
  • Complications: At higher risk for papillary thyroid carcinoma so discrete nodule should be biopsied
  • Treatment: Life-long L-thyroxine (T4)
  • Clinical: Usually present with hypothyroidism and goiter
