Pediatric Parotitis

  • Etiology: Acute parotitis typically caused by a virus such as paramyxovirus, Ebstein Barr, coxsackie, influenza A
  • Imaging of Acute Parotitis:
    — Common infectious cause of swollen lymph nodes of neck
    — Imaging US: Asymmetric enlargement of parotid gland without duct enlargement, hyperemia, may also see intraparotid lymph nodes
    — Imaging CT: Asymmetric enlargement of parotid gland without duct enlargement, hyperemia, may also see intraparotid lymph nodes, avid enhancement of parotid gland
  • Imaging of Recurrent Parotitis: Characterized by small anechoic spaces in parotid

  • Clinical: Mumps typically appears as parotitis and is usually bilateral but can be unilateral

Axial (above) CT with contrast of the face shows lymphoepithelial cysts in the parotid glands bilaterally. Coronal CT (below) shows the right parotid gland is enlarged and hyperemic and surrounded by inflammation. A right intraparotid lymph node is present.
