Pediatric Retroperitoneal Fasciitis

  • Etiology: Infection of retroperitonum that spreads through fascial planes and which may originate from intraperitoneal infections such as acute appendicitis
  • Imaging CT:
    — Retroperitoneal air
    — Asymmetrical retroperitoneal inflammation and enhancement
    — Non-focal retroperitoneal fluid collections including intrafascial collections
  • DDX:
  • Complications: Poor outcome
  • Treatment: Antibiotics and surgical debridgement

Axial (above), coronal (below left) and sagittal (below right) CT without contrast of the abdomen shows a large of amount of air in the right posterior pararenal space with associated fat stranding. The appendix was normal in size and there was an appendicolith that was outside of the appendix (not provided).
