    — Chronic stress on proximal patellar tendon which attaches on patella
    — Repetitive patellar tendon traction with quadriceps contraction at cartilaginous junction of inferior pole of patella
  • Imaging Radiograph: Avulsion fracture of inferior pole of patella

Sagittal T2 MRI of the knee shows mild thickening and increased signal in the proximal patellar tendon with adjacent bone marrow edema in the inferior pole of the patella and edema in Hoffa’s fat pad all secondary to an avulsion fracture of the inferior pole of the patella.
