Pediatric Sprue

  • Etiology: Malabsorptive gastrointestinal enteropathy
  • Imaging Small bowel follow through:
    — Small bowel dilatation proportional to disease severity
    — Barium flocculation due to intestinal hypersecretion
    — Jejunoileal fold pattern reversal
    — Hypersecretion and mucosal atrophy (moulage sign)
    — Transient non-obstructive intussusception, duodenal nodular changes
  • DDX:
  • Complications: If disease is uncontrolled there is increased risk of small bowel lymphoma and carcinoma
