Pediatric Vertebral Hemangioma

  • Etiology: Venous malformation
  • Imaging: Usually in thoracic spine
  • Imaging CT:
    — Axial CT shows polka dot or salt and pepper appearance due to thickened vertebral trabeculae
    — Sagittal CT shows corduroy sign due to thicker vertical trabeculae
  • Imaging MRI:
    — T1WI: Typical lipid-rich hemangioma has high signal while atypical lipid-poor hemangioma has low signal
    — T1WI post contrast: Significant enhancement due to high vascularity
    — T2WI: Bright signal usually greater than on T1 due to high water content
  • DDX:
  • Complications:
  • Treatment: Usually not necessary
  • Clinical: Usually asymptomatic and noticed incidentally
