AP image from an ERCP exam shows a beaded appearance to the biliary tree with multiple strictures causing multiple focal dilations of the biliary ducts. Overall, there is a pruned appearance to the intrahepatic biliary tree.

The diagnosis was primary sclerosing cholangitis.
