US of gallbladder hydrops
Transverse (above) and sagittal (below) US of the liver shows an extremely dilated gallbladder that is filled with anechoic fluid. Ascites is also present inferior to the liver on the sagittal image.

The diagnosis was gallbladder hydrops.
