Differential diagnosis of pediatric supratentorial brain tumors

Approach to the differential diagnosis of supratentorial brain tumors:

  • Most are glial tumors – astrocytoma (most common), oliogodendroglioma, glioblastoma
  • Peripherally located tumors that involve cortex, are low grade, may cause seizures – ganglioglioma, dysembryoplastic neuroepithelial tumor, pleomorphic xanthoastrocytoma, oliogodendroglioma
  • Peripherally located tumors that involve cortex, are high grade – desmoplastic infantile ganglioglioma
  • Primitive neuroectodermal tumor and ependymoma are less common
  • Focal cortical dysplasia is often difficult to definitively differentiate from a brain tumor on imaging
