Sagittal US of the left scrotum (above) shows a normal appearing left testicle. Transverse US of the right scrotum (below) shows a small round echogenic lesion with extensive posterior shadowing that was felt to represent the remnant of an infarcted and calcified testicle.

The diagnosis was right prenatal testicle torsion.
