Transverse US of the right scrotum (above) shows a heterogenous appearing testicle surrounded by a complex fluid collection. Spectral doppler US of the right testicle (below) shows no arterial flow in the right testicle.

The diagnosis was testicle torsion surrounded by a hematocele.
